Seven schools in the district of Kantō are constantly at war with each other, and the students within—both male and female—have become skilled fighters and constantly battle with rival schools. Their lives and fates are guided by strange jewels, called magatama, that contain the spirits and destinies of warriors from the Three Kingdoms period of China, and also change colour according to the ranking of its bearer. The ultimate purpose of each of these high-school fighters, known as Toushi, is to become stronger than all others and ascend to become the eponymous Ikki Tousen—a relatively obscure yojijukugo. Literally translated, it means one knight who is the match for a thousand (other knights) (the character "ki", for "knight" does not translate directly, as knights in the Western mold were unknown in Japan, but the sense is the same). The destinies of those who wear the magatama gems, though, have repeated time and time again for the past eighteen centuries—without fail—and the carnage is immense each time around.

Hiromu is a high school boy who has everything he could ever want: a normal life, friends, and a girlfriend. That is until one day Hiromu asks everyone where Tetsuya is and no one knows. All of his classmates think he is crazy for insisting that this Tetsuya person exists since no one has heard of him. Then on his way home from school Hiromu finally meets a boy who remembers Tetsuya and says that Tetsuya was "delicious" and this isn't the only strange occurrence. The plot, which takes place at the end of a fictional 19th century Europe, revolves around Allen Walker. Allen is a member of the Black Order, a group of Exorcists connected with the Vatican. Their mission is to stop The Millennium Earl, an evil being planning to cleanse the world by destroying all the humans in the manner of Noah's great flood.
The Exorcists themselves are special people gifted with the ability to control and use Innocence, a divine substance created in the ages past to combat the Earl's minions, the Akuma. It is said that there are exactly 109 units of Innocence scattered around the world. Once an Innocence finds its compatible person, it will evolve to become a weapon to fight the Akuma.

The main character, Karin Karino, is a bespectacled and shy girl who happens to meet two boys on a bus. The boys, from Takashiro (a local boys' high school), are named Tohru Ebihara and Aoi Kiriya. The boys, who are taking pictures of girls on the bus, initially make snide remarks (such as "four eyes") at Karin for her wearing glasses. Then Kiriya approaches her when he realizes that she reads a book called Blue, a book by a photographer named Yuuji. Karin is uneasy about the advances and starts to leave when the bus stops at the stop for Seika Girls' Academy, which is Karin's high school. Karin drops her book, and Kiriya inadvertently lifts Karin's skirt when he tries to give her book back. The skirt-lifting, combined with the fact that Kiriya still has his camera in his hand, causes Karin to think that Kiriya is "upskirting" her. Karin hits Kiriya and leaves without her book.
